East Jefferson General Hospital Sports Medicine
In order to meet Louisiana High School Athletic Association (LHSAA) eligibility requirements, it is mandatory for all students participating in Co-Curricular Athletics to have a LHSAA Medical History Evaluation and Physical Form completed and signed by a licensed Physician or Nurse Practitioner. To simplify this process, the Academy of the Sacred Heart Athletic Department and East Jefferson General Hospital Sports Medicine will be offering a comprehensive physical/orthopedic screening on campus for all students, currently in grades 4 - 11, who will be trying-out or participating on any athletic team or cheerleading. The physical will include a general medical evaluation by an East Jefferson doctor of family medicine, Dr. Clayton Mazoue and an orthopedic assessment and evaluation by Dr. Charles Haddad, Pontchartrain Bone and Joint Clinic. Dr. Haddad is an orthopedic surgeon specializing in sports medicine. During the 2010 - 2011 school year, both Dr. Haddad and Dr. Mazoue will be available, as needed, for consultation, evaluation and treatment of any injured student/athlete. The availability of Dr. Haddad and Dr. Mazoue, allows the Academy of the Sacred Heart to offer a comprehensive sports medicine program, headed by Jason Aucoin, M.A., ATC/L, to ensure the safety and health of all ASH student/athletes.
The physical/orthopedic screening will take place on Wednesday, May 12th beginning at 3:00pm. Refer to the schedule below to determine what time to report for your physical/orthopedic screening.

A through N: report to the back gym at 3:00pm
H through Z: report to the back gym at 3:45pm
Students need to wear T-shirts and shorts for the physical/orthopedic screening.
The fee for the physical/orthopedic screening is $25.00. This fee will be collected on Wednesday, May 12th. It can be paid in cash or by check, payable to 'Academy of the Sacred Heart'.
Important: Please log onto the ASH website and download the Athletic Forms. Bring your completed Athletic Forms to the Physical/Orthopedic Screening on May 12th. All Upper School and Middle School student/athletes must complete all of the Athletic Forms. Student/athletes will not be allowed to participate in athletics until all of the Athletic Forms are completed and on file in the office of the Athletic Director.
