Below is a collection of helpful reference information, uniform requirements and medical forms that you will be required to complete. Please have your physician complete all forms that pertain to your daughter(s).

Sacred Heart offers after-care and enrichment activities fo Little Hearts through Middle Grade students in a safe and supportive environment on campus. Summer Literacy and Written Language Enrichment

Summer Reading 2024 Award Titles
We invite your daughter to participate in Summer Enrichment. Summer Enrichment intends to provide students entering Pre-K through fourth grade with ways to engage in reading, writing, and math grade-level activities. Each student will be provided:
• A Grade-Level specific Summer Reading Book chosen by classroom teachers. We encourage parents and students to read this book over and over throughout the summer. The book will be used for a whole class activity at the start of the 2024-25 school year.
• Deck of Hearts Summer Enrichment Card Game- Each student will receive a customized deck of cards. The cards include summer enrichment choices including:
- Activities that incorporate Sacred Hearts Goals and Criteria
- Literacy and Written Language Activities (Incoming Kindergarten through Fourth Grade Students)
- Meaningful Math Practice (Incoming Kindergarten through Fourth Grade Students) and Fact Fluency for Grades 1-4
Summer Reading Options
Please stop by the Mater Campus Receptionist Desk when convenient. There is an envelope with your daughter's name with the book and deck of cards enclosed. Although Summer Enrichment is not mandatory, students are encouraged to participate to remain engaged with grade-level concepts.

PHYSICALS AT ASH

Cost: FREE (for all students participating in High School Athletics)
Note: Parents, please print this medical form. Then sign and date the form before your daughter comes to the physical.
In order to meet Louisiana High School Athletic Association (LHSAA) eligibility requirements, it is mandatory for all students participating in Co-Curricular Athletics to have a LHSAA Medical History Evaluation and Physical Form completed and signed by a licensed Physician or Nurse Practitioner. To simplify this process, the Academy of the Sacred Heart Athletic Department and Tulane Institute of Sports Medicine will be offering a comprehensive physical/orthopedic screening on campus for all students. The physical will include a general medical evaluation by a Tulane Institute of Sports Medicine doctor of family medicine and an orthopedic assessment/evaluation by an orthopedic at the Tulane Institute of Sports Medicine. During the 2024–2025 school year, the Tulane Institute of Sports Medicine will be available, as needed, for consultation, evaluation and treatment of any injured student/athlete. The availability of these orthopedics allows the Academy of the Sacred Heart to offer a comprehensive sports medicine program, headed by Bethany Perez, to ensure the safety and health of all ASH student/athletes.
This is a great opportunity for any athletes to get the proper medical form filled out and it will be completed before the summer. Anyone that needs camp physicals to be completed is also welcome to come. There will be an orthopedist on site to sign off on these physicals.
ALL ATHLETES must have an up-to-date physical prior to participation in a sport. The LHSAA requires a specific form that must be used for the physical for all 7th-12th grade JV and Varsity athletes.

DANCE TEAM TRY OUT

Details: Open to all rising 8-12 grade ASH Students

Where: ASH Dance Studio, 1st Floor, Favrot Arts Building; enter under the bridge on Carondelet St.
Dancers should wear appropriate, comfortable clothing and JAZZ SHOES. Plan to attend both days of tryouts. Come ready to move! Tryouts will be a chance for aspiring team members to see how we practice and work together throughout the season. So, if you’re not sure about what the ASH Dance Team is like, this is a great way to find out! On Monday, we will warm up, and the officer candidates will teach a short dance. After warm up on Tuesday, dancers will perform the dance in small groups, show individual skills (leaps, turns, etc), and officer candidates will answer additional questions.
ASH Dance Team generally practices on Thursdays from 3:15-5:00pm throughout the school year. The practice schedule shifts as we enter basketball season. The Dance Team performs at half-time for all varsity home basketball games, marches in the Krewe of Carrollton parade, hosts a lower and middle school clinic, performs for pep rallies and other school events, and holds a Spring Dance Concert.
