I hereby grant to Academy of the Sacred Heart the irrevocable and unrestricted right to identify my child for Athletics Department purposes when any and all published photographcs, videos, and other images in which my daughter, the attending student, may be included, or images in which I may be included, now existing or hereafter made, in any case, with our without identifying me (except as may be indicated below) for editorial, advertising, news, social media or any other purpose and in any manner and medium; to alter the same without restriction; and to copyright the same. I hereby release Academy of the Sacred Heart and its officers, directors, agent employees, and contractors from all claims and liabilities in any way arising out of or in connection with or relating to such photographs, videos, images, full names, and quotations.
